A Case of Reversible Cerebral Vasoconstriction Syndrome without Headache

Abstract

|
Reversible cerebral vasoconstriction syndrome (RCVS) is a cerebrovascular disorder associated with multifocal arterial constriction and variable neurological symptoms. The primary clinical manifestation is a recurrent suddenonset and severe (so called Thunderclap) headache over 1-2 weeks. Diagnosis requires demonstration of the characteristic diffuse segmental vasoconstriction on cerebral vascular imaging with resolution within a few days to 3 months. We report a case of 69 year-old woman who showed segmental vasoconstriction on cerebral vascular imaging without any headache. Although recurrent thunderclap headache is the hallmark of RCVS, the absence of such a pattern should not lead to discard the diagnosis.
